Influenza vaccinations offered for Trenton R-IX Students, grades 5 - 12
***Forms must be returned to school by Friday, October 5, if students and staff would like to receive the vaccination.***
The Grundy County Health Department will be partnering with the Trenton R-IX School district to offer the influenza vaccine to students and staff members at the end of October. Attached you will find a consent and history form (that must be returned) and a form of screening questions. Please be sure to include insurance information on the consent and history sheet. Check with your insurance company regarding any out of pocket costs. We will be ordering them based on the number of consents we receive. No child will receive the vaccine unless the consent and history form is filled out completely and signed. The form must be turned in to the school by October 5th. The date the vaccines will be given will be determined once the forms have been returned. Please call the school nurse with any questions or concerns at 359-4328.
